The Bravo box floats and chains in the UDC 1/2 (east & west) and the UDC 5/6 (east) sumps <br />failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be <br />maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it <br />detects a leak. The service technician adjusted the floats and chains and verified functionality. <br />REPEAT VIOLATION <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by May 7, 2014 along <br />with a statement documenting the corrective actions that have been or will be taken for each violation <br />and any supporting paperwork. <br />Note: The complete Return to Compliance paperwork for the April 1, 2013 inspection was not submitted. <br />A deficiency letter was issued on November 26, 2013.
