El Dorado Street <br />Stockton, CA 95206 <br />DIRECTOR <br />Donna Heran, RENS <br />ASSISTANT DIRECTOR <br />Laurie Cotulla, REHS <br />PROGRAM COORDINATORS <br />Carl Borgman, REHS <br />Mike Huggins, REHS, RDI <br />Margaret Lagorio, REHS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />On February 22, 2006, Ms. Michelle Le, Senior Registered Environmental Health <br />Specialist (REHS), San Joaquin County Environmental Health Department (EHD), sent a <br />certified letter to the above referenced facility stating that an annual underground storage <br />tank (UST) monitoring system certification (due by July 2005) and the triennial UST <br />secondary containment testing (due by December 2005) were both past due (Attachment <br />1). <br />On March 21, 2006, secondary containment testing was performed and a test report was <br />submitted to this office on April 21, 2006 (Attachment 2). <br />On October 30, 2006, Mr. Ray von Fliie, Senior REHS, EHD, conducted a routine UST <br />inspection at the above referenced facility. The annual monitoring system certification, <br />leak detector testing, and spill container testing were over two years late. While <br />reviewing the facility's files, Mr. von Flue found that an UST monitoring system <br />certification had been conducted on December 27, 2005, by Mr. Ed Stearns with Expert <br />Environmental. Notification of the December 2005 test was not provided to this office, <br />and the test report was rejected. Mr. von Flue also found that current financial <br />responsibility documents had not been submitted to this office, which was a repeat <br />violation from the last routine UST inspection on July 14, 2004. The monitoring plan <br />was not current, and alarm monitoring records were not being kept (Attachment 3). <br />Mr. von Flue also performed a routine hazardous waste inspection on October 30, 2006. <br />No manifests for the disposal of hazardous waste were found on site.
