EI Dorado St., Stockton <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />Linda Turkatte, REHS <br />SR0064379 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit for temporary closure of the following underground storage tank <br />(UST) systems, including piping and associated appurtenances, at the above noted address: <br />Tank # 39-0515512: 3,000 gallon premium (91) unleaded gasoline <br />Tank # 39-0515513: 9,000 gallon regular (87) unleaded gasoline <br />The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). <br />a. Decontamination and inertion of the tanks <br />b. Any subsequent inertion of the tanks after the initial inspection <br />c. Quarterly inspections: EHD must be present to witness all quarterly inspections. <br />Please be aware that all EHD staff time associated with these inspections will be <br />billed at the current hourly rate ($125). <br />4. Inert USTs with 30 pounds of dry ice per 1,000 gallon volume after triple rinsing. Be <br />aware, additional testing/monitoring to ensure tanks remain inerted may be required by <br />the Stockton Fire Department. <br />5. A copy of the submitted "Health and Safety Plan" must be on the jobsite for review. <br />
