The maintenance and monitoring records for this facility were not found on site. These records <br /> shall be kept on site for at least 3 years. Immediately locate and maintain the maintenance and <br /> monitoring records on site and submit copies to the EHD by December 21, 2012. <br /> 202. Approved copies of the monitoring and response plans were not found on site. Copies of these <br /> plans shall be retained on site at all times. Immediately locate and retain a copy on site. <br /> 202. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and <br /> all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit <br /> and retain it on site. <br /> 205. The designated operator monthly inspection reports for the last twelve months were not found on <br /> site. The most recent designated operator report is dated May 24, 2011. Designated operator monthly <br /> inspection reports for the previous twelve months shall be retained on site. Locate and ensure that <br /> copies of the previous twelve months of designated operator monthly inspection reports are maintained <br /> on site. Submit copies to the EHD by December 21, 2012. <br /> 206. The designated operator employee training has not been performed since prior to May 2011. The <br /> designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months. The training shall include, but is <br /> not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
