During the monthly inspection,the designated operator shall check that all <br /> required testing and maintenance for the UST system have been completed and document the dates they were <br /> done. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. The service technician on site,who is also a designated operator for the facility <br /> corrected the test date. This was corrected on site. <br /> The designated operator failed to document and check that the following alarms were responded to appropriately: <br /> L15 91 Fill Sump Sensor Out Alarm on 3/29/17 <br /> Q1 91 Octane PLLD Shutdown and Q2 87 Master PLLD Shutdown Alarms on 3/29/17, 3/31/17, and 5/26/17 <br /> The designated operator documented, but failed to check that the following alarms were responded to appropriately: <br /> Q1 91 Octaine PLLD Shutdown Alarms on 4/6/17,4/7/17,4/9/17 and 4/24/17 <br /> Q2 87 Master PLLD Shutdown Alarms on 4/6/17,4/7/17, and 4/9/17 <br /> L1 Dispenser 1/2 Sensor Out Alarms on 4/7/17 and 9/17/17 <br /> Sensor Out Alarms for L1 Dispenser 1/2, L11 87 3 STP, L14 91 STP, and L15 91 Fill on 5/22/17 <br /> Q1 91 Octane PLLD Shutdown and Q2 87 Master PLLD Shutdown Alarms on 5/22/17 <br /> T2(Unleaded Center)Overfill and High Product Alarms and T3(Unleaded East)Overfill Alarm on 8/28/2017 <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately,and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The bottom of the 91-product fill sump 323 sensor was taped, preventing the sensor from detecting a potential leak <br /> at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. The tape was removed from the sensor.The sensor was tested, passed, and relocated to the <br /> lowest point of the sump by the service technician.
