Reviewed installation <br /> plan. <br /> 3 . 6/14/96 Called Mr. Krukar of Ultramar regarding the <br /> deficiencies . Talked with Mr. Unruh regarding the <br /> deficiencies. <br /> 4 . 6/17/96 Talked with Mr. Krukar regarding the plan status <br /> and deficiencies . At the request of Mr. Krukar, the plan <br /> reviewed during and after business hours . <br /> 5 . 6/18/96 reviewed plan with supervisor. Called Mr. Unruh <br /> and Mr. Krukar regarding the flex boots . Reviewed other two <br /> applications since they were not copies and were hand- <br /> written. <br /> 6 . 6/19/96 worked on plans . Discussed UL-listed flex boots <br /> and other issues with Mr. Stephenson. Met with Mr. Walton <br /> and discussed the conditions of the permit approval with <br /> him. <br /> 7 . 6/21/96 visited site for tank setting. One of the tank <br /> lost vacuum. Waited for repair and retest before setting <br /> the tanks . <br /> 8 . 7/3/96 visited site for primary piping and secondary tank <br /> test . There was a leak in prem. UL pipe and one of the <br /> secondary tanks . The retest for piping also failed. <br /> 9 . 7/5/96 retesting the piping failed. Retested the piping <br /> again after pipe was repaired. <br /> 10 . 7/9/96 visited site for inspection on secondary pipes . <br /> primary tanks, corrosion protection and the start of 24 hour <br /> lake test on sumps . Only secondary pipes were ready for the <br /> inspection upon my arrival . Returned to site for other <br /> inspections . Sumps were ready for inspection at time of <br /> arrival . <br /> 11 . 7/10/96 visited site to finish the 24 hour lake test and <br /> for overfill performance test . <br /> 12 . 7/11/96 completed paper work. <br /> 13 . 8/5/96 conducted final inspection. The float switches for <br /> impact valve of some dispensers did not work. The automatic <br /> pump shut-off did not function. Revisited site for <br /> inspection on automatic pump shut-off . <br /> 14 . 8/6/96 completed paper work for final inspection and permit <br /> approval . <br /> Please note that the fee you have paid includes $510 for new UST <br /> permit and $168 for state surcharge .
