The 91 product tank annular liquid sensor was replaced on December 23, 2011 according to <br />facility records and a permit was not obtained from the EHD. Emergency repairs relating to UST system <br />monitoring and release detection may be performed by a properly licensed, trained, and certified <br />contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. <br />All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. <br />Immediately obtain a permit from the EHD for the 91 tank annular sensor replacement. A "Failure to <br />Obtain a Permit" penalty applies. A $750 charge applies: $375 permit fee + $375 penalty. <br />305. The dispenser 1/2 sump Bravo float and chain (middle location) failed to stop the flow of product at <br />the dispenser when tested. The float was cracked and was not air tight. All monitoring equipment shall <br />be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it <br />detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor <br />replace the failed component with a functional component (LG 113-x listed, if applicable) and obtain a <br />permit within one business day from the EHD. If the failed component can't be replaced immediately, <br />there is a possibility that the associated UST system may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD 23-02/ Rev 04/19/12 Page 3 UST INSPECTION REPORT <br />
