This information was required to be submitted by June 16, 2016 . <br />Although the Environmental Health Department (EHD) received the Return to Compliance Certification (RTC) form <br />dated 6/6/16 and a site plan that was incomplete, there was no documentation or narrative included. This <br />information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br />inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br />actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br />corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. The outstanding <br />violations include: UST Tank information is not current; plot plan failed to completely show where monitoring is <br />performed; and failed to inform the EHD of a change of designated operator within 30 days. <br />This is a Class II violation. <br />205 <br />CCR 2712(i) Failure to have an approved UST response plan available onsite. <br />An approved copy of the Response Plan was not available for inspection. A copy of the Response Plan shall be on <br />site at all times. Immediately locate and retain a copy on site. <br />This is a minor violation. <br />313 <br />HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product -tight. <br />Seepage was observed at and below two of the shear valves in under dispenser containment (UDC) 7-8 and <br />approximately one cup of liquid was observed at the bottom of the UDC, indicating a leak in the primary piping. All <br />primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br />contractor repair or replace the failed components. <br />This is a repeat violation, Class II. <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Approximately three cups of liquid were observed in the diesel submersible turbine pump (STP) sump, a trace of <br />liquid was observed in the 87 -product STP sump, approximately one-quarter cup of liquid was observed in under <br />dispenser containments (UDC) 1-2. and approximately one cup of liquid was observed in UDC 7-8. If water could <br />enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br />The service technician removed the liquid from the diesel and 87 -product STP sumps and UDCs 1-2 and 7-8.. <br />Ensure that all sumps and annular spaces are maintained free of liquid. <br />This was corrected on site. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by June 8, 2017. <br />FA0003611 PR0231127 SCO01 05/09/2017 <br />EHD 23-01 Rev. 09/22/16 Page 5 of 6 Underground Storage Tank Program OIR <br />
