This is a repeat violation, Class II. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. UST Tank Information forms for underground storage tanks TA0508150 (Regular unleaded) and TA0508152 (diesel) are not current in CERS. For TA0508150 (Regular unleaded) -beneath Product/Waste Piping Construction enter "Fiberglass" for Primary Containment and "Fiberglass" for Secondary Containment and enter "Single -walled" for Piping/Turbine Containment Sump; and beneath Vent/Vapor Recovery (VR) and Riser/Fill Pipe Piping Construction, remove "single -walled" from Vent Piping Transition Sumps. For TA0508152 (diesel) -beneath Product/Waste Piping Construction, remove "steel" and replace this with "fiberglass" for Secondary Containment, and enter "Single -walled" for Piping Turbine Containment Sump. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a repeat violation, Class II. 105 CCR 2632(d)(1) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. For TA0508150 (Regular unleaded), beneath Recordkeeping, change "no" to "yes" for Equipment Maintenance and Calibration Records. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 106 CCR 2632(d)(2) Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. Add sensors to dispensers 5-6 and 7-8. Add float -and -chains to dispensers 1-2 and 3-4. Label the diesel submersible turbine pump (STP) sump with a sensor and and mechanical line leak detector. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II.
