Under "Training" change "no" to "yes" for <br />Designated Operator Training for both underground storage tanks. The monitoring plan must be uploaded to the <br />California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and <br />submit for review by the EHD. <br />This is a minor violation. <br />203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br />Records of last year's May 6, 2015 monitoring system certification contractor's report as well as the secondary <br />containment testing report completed on May 1, 2015 were not found on site. These records shall be kept on site for <br />the life of the tank. During the inspection today, the facility was able to locate last year's May 6, 2015 monitoring <br />system certification report from the contractor. Immediately locate and maintain all other records on site and submit <br />copies to the EHD. <br />This is a minor violation. <br />207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator, Harold Largo, failed to list the correct ICC California UST System Operator expiration date on <br />the monthly inspection reports for March and April 2016, and May, June, July, September, November and December <br />2015. The correct expiration date is 12/4/2016. Also the designated operator, Curtis Carpenter, listed the ICC <br />California UST System Operator expiration date as March 20, 2015 on the August 3, 2015 monthly inspection report. <br />During the monthly inspection, the designated operator shall verify that all information is correct on the monthly <br />designated operator inspection reports. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />This is a minor violation. <br />209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training list did not include the "HIRE DATE" for most of the employees. The <br />designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This was corrected on site. <br />This is a minor violation. <br />Page 4 of 6 <br />
