San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sl oq v_org/ehd <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />VAN DE POL ENT INCIPACIFIC PRIDE <br />Facility Address: <br />351 N BECKMAN RD LODI <br />Date: <br />June 27 2017 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Approximately two inches deep of liquid was observed in the under dispenser containment (UDC) for dispenser 5/6. If <br />water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of <br />properly. The company representative Ted Shackelford removed approximately two gallons of liquid from UDC 5/6. No <br />fuel piping leaks were observed. Ensure that all sumps and annular spaces are maintained free of liquid. Violation <br />resolved during the inspection. <br />This is a minor violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br />complete report which replaces the initial checklist. <br />A permit to operate the UST system has not been issued for 2017. Once the violations in this report are resolved <br />the 2017 UST operating permit will be issued to the tank operator. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by August 3, 2017. <br />AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS <br />FACILITY BACK INTO COMPLIANCE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Received by: Date: <br />Inspector: <br />Inspector Phone: <br />Printed Name and Title: <br />GARRETT BACKUS, Lead Senior REHS <br />(209) 468-2986 <br />Page 5 of 5 <br />
