Notification of Zane as one <br />of the site designated operators has not been provided in CERS. Any changes shall be reported to the EHD within 30 <br />days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and <br />upload the notification identifying all the designated operators for this facility in the Owner Statement of Designated <br />UST Operator Compliance location. <br />This is a minor violation. <br />210 <br />CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The January, February, March, April, and May 2017 designated operator monthly inspection reports were not found on <br />site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. The <br />designated operator provided copies of the reports by email to the EHD and the operator on 6-27-17 after the <br />inspection. Violation resolved. <br />This is a minor violation. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The under dispenser containment 5/6 sump sensor was raised approximately 4-5 inches off the lowest point of the <br />sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be <br />able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump by <br />the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. <br />Violation resolved during the inspection. <br />This is a Class II violation. <br />309 <br />CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br />The 87 product turbine sump liquid sensor (middle tank of 5 tanks) failed to activate an audible and visual alarm when <br />tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate <br />an audible and visual alarm when it detects a leak. Correct immediately by having a properly licensed, trained, and <br />certified contractor replace the failed component with a functional component (LG 113-x listed, if applicable). Provide <br />notification to the EHD with the following information for like -for -like replacements: Technician company name, ICC <br />service technician certification, Veeder Root certification, brand and model number of the new sensor, time and date of <br />sensor installation. Apply for a repair permit with the EHD within one business day for replacements that are not <br />like -for -like. If the failed component can't be replaced immediately, there is a possibility that the 87 octane UST <br />system may be red tagged to prevent fuel inputs. <br />This is a minor violation. <br />Page 4 of 5 <br />
