The <br />alarm responses are lacking for the following: 3-31-16 L1 Red Diesel Fill fuel alarm and 11-24-15 L2 91 Fill alarm. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />318 <br />CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br />a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. <br />This is a repeat violation, Class II. <br />2010 <br />CCR 2712(i) Prior to the routine inspection, Marty Thorpe called to inform the SJCEHD that concrete had been broken <br />and replaced over the 91 UST and that a permit had not been obtained for this work. A copy of the permit and all <br />conditions and attachments, including monitoring plans, shall be retained at the facility. Construction, repair and/or <br />removal permits are required from the EHD prior to any change, repair, or removal of UST system equipment. A failure <br />to obtain permit penalty (double permit fee) applies. Correct immediately by including the failure to obtain permit <br />penalty fee with your permit application. <br />This is a minor violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection.
