UST Facility Operating Permit Application and UST Tank Information for all USTs at this facility are <br />not current in CERS. Any change of information must be updated in CERS within 30 days of the <br />changes. Refer to the facility CERS account which identifies why it was not accepted and the items that <br />need to be corrected. Complete a separate tank form for each tank.
