Beckman Rd. Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />107. The plot plan/site map has not been uploaded on CERS. The plot plan/site map must be submitted <br />on CERS and completely show where monitoring is performed. Log into CERS, upload the completed <br />form, and submit for review by the EHD. Maintain an approved copy on site. <br />109 & 110. Annual monitoring system certification, leak detector testing, and spill container testing were <br />last performed on 6-28-13 and was due by June 2014. Testing has not been completed for the Red <br />diesel annular sensor, UDC 9/10 sensor, diesel B turbine positive shutdown, and diesel B turbine leak <br />detector. These tests are required once every 12 months. Immediately schedule these tests and <br />provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at <br />our current hourly rate. <br />203. Diesel filters in dispensers 1/2 and 3/4 are weeping diesel and was not recorded. Any release from <br />the primary containment that does not escape the secondary containment and was cleaned up within <br />eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EHD by 7-30-14. <br />204. January and March 2014 and October, November, and December 2013 did not have marked UDC <br />9/10 as being inspected. The designated operator shall perform monthly visual inspections of all <br />underground storage tank systems for which they are designated. The results of each inspection shall <br />be recorded in a monthly inspection report. The D.O. shall inspect for the presence of hazardous <br />substance, water, or debris in under -dispenser containment area, and checking that the monitoring <br />equipment in these areas are located in the proper position to detect a leak at the earliest opportunity. <br />Ensure that designated operators performing monthly inspections at this facility are inspecting all of the <br />required areas and noting it on the reports. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT_10 REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Receive <br />Title: <br />Aris Veloso (209) 468-3453 <br />S Qc\/,� <br />EHD 23-02-003 Rev 04/19/12 % CONTINUATION FORM <br />
