The designated operator failed to document responses to all the alarms from the attached alarm history on the 10-4-13 and 8-1-13 designated operator monthly inspection reports. The missing alarm responses are lacking for the L2 91 fill sump sensor 9-21-13 alarm, L7 91 turbine sump sensor 9-21-13 alarm, L11 Red diesel annular sensor 7-24-13 alarm, and L14 diesel annular sensor alarm. Alarm history reports attached to the designated operator reports were incomplete. The 4-10-14 D.O. report lacked the C1 87 annular alarm history print out. The 1-20-14 D.O. report lacked the L6 Red diesel turbine sump alarm history print out. The 12-2-13 D.O. report lacked the L4 diesel fill, L7 91 turbine sump, L9 diesel turbine sump, L14 diesel annular, and C1 87 annular alarm history reports. The 7-1-13 D.O. report lacked the entire alarm history report. The During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 306. A fuel leak was found at the diesel filters in dispensers 1/2 and 3/4. All primary containment must be product tight. Immediately have a properly licensed, trained, and certified contractor replace or tighten the fuel filter. 311. The 91 and diesel "A" line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. 311. The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, there is a possibility that the 87 UST system may be red tagged to prevent fuel inputs. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
