The designated operator failed to document responses to all the alarms from the attached alarm <br />history on the 10-4-13 and 8-1-13 designated operator monthly inspection reports. The missing alarm <br />responses are lacking for the L2 91 fill sump sensor 9-21-13 alarm, L7 91 turbine sump sensor 9-21-13 <br />alarm, L11 Red diesel annular sensor 7-24-13 alarm, and L14 diesel annular sensor alarm. Alarm history <br />reports attached to the designated operator reports were incomplete. The 4-10-14 D.O. report lacked the <br />C1 87 annular alarm history print out. The 1-20-14 D.O. report lacked the L6 Red diesel turbine sump <br />alarm history print out. The 12-2-13 D.O. report lacked the L4 diesel fill, L7 91 turbine sump, L9 diesel <br />turbine sump, L14 diesel annular, and C1 87 annular alarm history reports. The 7-1-13 D.O. report <br />lacked the entire alarm history report. The During the monthly inspection, the designated operator shall <br />review the alarm history for the previous month, check that each alarm was documented and responded <br />to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />306. A fuel leak was found at the diesel filters in dispensers 1/2 and 3/4. All primary containment must <br />be product tight. Immediately have a properly licensed, trained, and certified contractor replace or <br />tighten the fuel filter. <br />311. The 91 and diesel "A" line leak detector failed to detect a leak when tested. All line leak detectors <br />shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak <br />detector, retested it, and it passed. <br />311. The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br />obtain a permit within one business day from the EHD.
