CLASS 11. or MINOR -Notice to ComDIv) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />201. The L21 dispenser #9/10 sensor alarmed on December 26, 2012. The designated operator did not <br />document the response to this alarm on the monthly designated operator report nor was this alarm <br />documented on the facility alarm log. Alarm maintenance records for these alarms were not found on <br />site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the facility <br />alarm log and alarm maintenance records on site and submit copies to the EHD by July 28, 2013. <br />204. The L21 dispenser #9/10 sensor alarmed on December 26, 2012. The designated operator did not <br />document the response to this alarm on the monthly designated operator report. During the monthly <br />inspection, the designated operator shall review the alarm history for the previous month, check that <br />each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. <br />204. The designated operator(D.O.) failed to check and note the correct test dates on the monthly D.O. <br />reports. The May 2013 D.O. lists the monitoring certification, leak detector, and spill container tests as <br />June 27, 2011, but should have been listed as June 25, 2012. The February, March, April, and August <br />2012 D.O. reports list the monitoring certification, leak detector, and spill container tests as June 27, <br />2012, but should have been listed as June 25, 2012. The secondary containment test date for these <br />reports was listed as August 26, 2012, but should be August 26, 2011. The January 2013 and July 2012 <br />D.O. reports list the monitoring certification, leak detector, and spill container tests as June 22, 2012, but <br />should have been listed as June 25, 2012. The June and September 2012 D.O. reports list the <br />monitoring certification, leak detector, and spill container tests as June 27, 2011, but should have been <br />listed as June 25, 2012. During the monthly inspection, the designated operator shall check that all <br />required testing and maintenance for the UST system have been completed and document the dates <br />they were done. Ensure that designated operator performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />I ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
