Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS 11 CLASS II, or MINOR -Notice to Comply) <br />303. During the inspection Carl Wayne Henderson installed a new Veeder Root 0794390-420 sensor <br />(serial # 837699) in Red Dye Diesel Tank annular space and this sensor not located to detect a leak at <br />the earliest opportunity. The existing annular sensor is stuck at the bottom of the annular space. Carl <br />Wayne Henderson states that the newly installed sensor is prevented from being placed at the lowest <br />point to detect a leak at the earliest opportunity because of the existing sensor. Monitoring equipment <br />shall be maintained to be able to detect a leak at the earliest possible opportunity. Carl Wayne <br />Henderson states that he will obtain a permit for the sensor he installed so that EHD can re -inspect and <br />witness the removal of the existing sensor and placement of the new sensor at the lowest point to detect <br />a leak at the earliest opportunity. Carl Wayne Henderson also states that he will also install the gel pack <br />for the spliced wires inside the j -box during the final inspection. Take the necessary precautions to <br />ensure that all sensors are located at the lowest point to detect a leak at the earliest opportunity. <br />Immediately submit the permit application for the newly installed sensor. <br />306. A fuel leak was found at the diesel filters inside dispensers #1/2 and #3/4. All primary containment <br />must be product tight. Have a properly licensed, trained, and certified contractor tighten the diesel <br />filters. If the failed component can't be replaced immediately, there is a possibility that the diesel #A <br />UST system may be red tagged to prevent fuel inputs. <br />104. Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by July 28, 2013. <br />113. A new designated operator Ed Stearns was hired on November 12, 2012, and notification was not <br />provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 <br />days. Submit notification to the EHD identifying all the designated operators for this facility by July 28, <br />2013. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by July 28, 2013. <br />Ensure that all required information has been entered into the California Electronic Reporting System. <br />This completed report supersedes the original checklist issued to the operator during the inspection. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Aris Cacapit (209) 468-3453 '0%� <br />Received By: <br />�b <br />Title: <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
