Ensure that this facility documents all daily alarms from the Veeder Root panel and <br /> responds to each alarm appropriately. <br /> Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration <br /> and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in <br /> the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test <br /> results; (6) results of inventory readings and reconciliations. <br /> Send in information to EHD describing how the alarms were responded to. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the 12/20/2016 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include 2 liquid warning alarms on 11/20/2016 and 12/12/2016 for the H4 piping vault sensor. The <br /> H4(piping vault sensor)and H5(fuel polisher sensor)were not printed onto the 12/20/2017 designated operator <br /> inspection report. <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> The designated operator on site printed out the alarms and addressed them on the designated operator monthly <br /> inspection report. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 214 HSC 25281.5(b)(3) Unburied emergency generator fuel piping not visually inspected at least monthly or log kept. <br /> A log of monthly checks of the unburied single walled piping was not found on site. According to the designated <br /> operator(DO)on site, a company comes out to inspect the generator. The DO did not have the documents. For an <br /> emergency generator tank system,the unburied piping is exempt from regulation if visual inspections are made <br /> each time the system is operated, but no less than monthly, and a log is maintained.
