(CLASS 1, CLASS 11, or MINOR - Notice to'Comply) <br />Item # <br />Remarks <br />206 <br />CCR 2712 Failure to comply with UST operating permit conditions. <br />According to work orders on site, a cold start of the monitoring panel was performed on 2/17/17. No notification <br />was made to the EHD for this work. Repairs relating to UST system monitoring and release detection may be <br />performed by a properly licensed, trained, and certified contractor and notification made pursuant to the EHD'S <br />policy. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. <br />Immediately obtain a permit from the EHD for the cold start of the monitoring panel. A "Failure to Obtain a Permit" <br />penalty applies. <br />This is a repeat violation, Class II. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Hamgi Elgahin was not performed. At the time of inspection, Mr. <br />Elgahin was the only worker on site. He said he is not really an employee, but is just helping out for a couple weeks. <br />The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a repeat violation, Class II. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The 91 STP sump sensor was observed placed on the side opposite the fuel piping and not located to detect a leak <br />at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br />possible opportunity. The sensor was relocated to the fuel piping side by the service technician. Ensure that all <br />monitoring equipment is maintained to detect a leak at the earliest opportunity. <br />This was corrected on site. <br />This is a repeat violation, Class 11. <br />309 <br />CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br />Both Bravo box floats and chains in the UDC 1/2 sump failed to stop the flow of product at the dispenser when <br />tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified <br />functionality. <br />This was corrected on site. <br />This is a repeat violation, Class 11. <br />Page 6 of 8 <br />
