UST Facility Operating Permit Application and UST Tank Information for all existing site tanks are <br />not current in CERS; no overfill prevention method is identified. Immediately log into CERS, update the <br />required information, and submit for review by the EHD. Complete a separate tank form for each tank. <br />Complete and submit a copy of the Return to Compliance Certification form with a separate statement of <br />how each violation was corrected, including supporting documentation, by September 8, 2013, the <br />response to the newly added violations 109, 110, and 111 could be submitted to the EHD by September <br />16, 2013. <br />Note: Upon the inspector's arrival at the facility (inspector arrived prior to the scheduled start of the <br />monitoring system certification) it was noted that all the sump sensors were already lifted.
