An"Underground Storage Tank Certification of Installation/Modification" form(Form <br /> C) shall be submitted by the owner or the owners'agent [pursuant to(CCR)2635(d)] <br /> before this upgrade will be considered complete by EHD. <br /> 8. Submit an updated Unified Program Consolidated Form for UST(Form B)and <br /> Monitoring Plan within 30 days of the completion of the work. <br /> 9. Permit expires 180 days from date of approval.
