The Contractor will stotify VERIZON at once and VERIZON will be responsible <br />for filing a report with the responsible State agency." <br />911111111111111K�� �#11131MVN <br />5.1 All work activity described herein relates to the installIation of the new UST system listed <br />in Section One under Site Location. <br />5.2 The Contractor shall have all necessaty approved and current licenses for the necessary <br />USTIAST work described herein as required by the State of Cakfanxia and all Local <br />Agencies that have lavM jurisdiction ever the enforcement of the corrective actions <br />being taken as outlined in this SOW. <br />5.3 The Contractor shall be responsible to insure that ALL mbcontractors assisting m. the <br />completion of this SOW meet ail of the State of California and local Environmental <br />controls, hoensing and training requirements for all phrases of their urvolvement, as well <br />5.4 The Contractor shall] not be allowed to work on site without a VERIZON Field Engineer <br />present <br />5.5 The Contractor shall be responsible for contacting all utilities for a "DIG ALERT" before <br />any construction begins requiring excavation. The Ca or shall be responsible for ail <br />damages caused as a result of their actions. <br />5.6 The Contractor shall perform AU work, furnish all materials and supply all labor and <br />equipment necessary to accomplish the descnl)ed tasks in this SOW. The ordering of the <br />VMMN furnished material and delivery will be coordinated and the responsibility of <br />the Construction Supervisor. <br />5.7 The Contractor shall research and develop all necessary drawings of the property as <br />=qWred to obtain permits for the installation of the UST/AST at this site. This includes, <br />but not hunted to, gas hues, telephone hoes, fiber ophc cables (VERIZON & other) water <br />lines, power hues, etc that are needed to obtain the permits for the corrective actions <br />covered in this SOW. <br />5.8 The Contractor " not begin construction on this site without the appropriate approvals <br />frim the responsible governmental agencies involved in USTIAST construction work. <br />VERIZON will not allow work to begin on this site until ALL PERUM have been <br />obtained and Faxed to 1972] 729-6513 to the Construction Supervisor for approval. <br />5.9 The Contractor shall remove and dispose of all excess materials, soil, gravel, etc., in <br />accordance with federal, state and county regulations VERIZON does not allow any <br />abandonment in place without prior approval and docurnentation. <br />5.10 Allo required for conduit or other work to penetrate concrete or block walls or <br />dabs shall be care drilled; such openings " be cut with standard cutting tools and shall <br />be cut to the mititaut size for ft conduit to pass through. All openings shall be patched <br />with materials that match existing materials and meet their original fire rating and <br />
