Secondary containment testing was performed on June 9, 2011, and a test report was received by <br />the EHD on July 21, 2011. A copy of the test report was not submitted to the EHD within 30 days of the <br />test. Ensure that a copy of the test results are submitted to the EHD within 30 days of the test. <br />109. Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on June 14, 2012, and a test report was not received within thirty days by the EHD. The test <br />report was received on July 25, 2012. A copy of the test report must be submitted within 30 days of the <br />test. Take the necessary precautions to ensure that a copy of the test report is submitted to the EHD <br />within thirty days of the test. <br />202. Current approved copies of the response plan was not found on site. The monitoring and response <br />plans must be current and approved by the EHD. An approved copy was copied during the inspection <br />and provided to operator. Continue to ensure that the required documentation is maintained on site. <br />303. The diesel turbine sump sensor was located on the lowest point of the sump, but on the opposite <br />side of the sump from where the product line penetrates the sump wall. This sensor is not located to <br />detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a <br />leak at the earliest possible opportunity. During the inspection, the technician repositioned the sensor to <br />the lowest point of the sump, on the side where the product line penetrates the sump wall, and is now <br />able to detect a leak at the earliest opportunity. Take the necessary precautions to ensure that all <br />sensors are located at the lowest point to detect a leak at the earliest opportunity. <br />306. A fuel leak was found at the diesel meters in the dispensers 9/10 and 11/12. All primary <br />containment must be product tight. Immediately have a properly licensed, trained, and certified <br />contractor replace or repair the failed component. Depending on the scope of work, a permit may be <br />necessary.
