Alarms were caused during annular sensor testing. History also shows L3 north sump fuel alarm <br />on 7-31-07 @ 10:14am and sensor out alarms on 7-31-07 @ 10:03am and 10:14am. L3 alarms were cleared when water <br />was pumped from sump. Documentation not yet available. <br />Item #9 - Documentation not available for monitoring system certification performed on 7-13-07. <br />Item #10 - Documentation not available for SB989 secondary containment testing. <br />Item #11 - Spill bucket failed testing on 7-13-07. Ticket LT -5029 was opened for repair or <br />replacement of spill bucket. <br />LT -5029 also addresses replacement of TLS 300C with TLS 350 to facilitate monitoring by AT&T Dallas. <br />Posted statement concerning L4 alarms on 4-30-07 - alarms were caused during engine radiator maintenance (by Holt of <br />California) when antifreeze entered the trench and sensor was moved to facilitate liquid removal. <br />items Ke uirin Follow -Up Actions: <br />Obtain and post documentation for: <br />- monitoring system certification. <br />- SB989 secondary containment testing. <br />- spill bucket testing. <br />- L2 alarm activity on 8-3-07. <br />- L3 alarm activity on 7-31-07. <br />Perform required repairs and provide documentation for repairs and testing. <br />Instructions: <br />Monthly visual inspection of the UST system must be conducted by a Designated UST Operator, who <br />possesses a current California UST System Operator certification issued by the International Code Council. <br />A copy of this monthly visual inspection checklist must be provided to the UST Owner or Operator. <br />The Designated UST Operator must alert the UST Owner or Operator of any condition discovered during the monthly visual <br />inspection that may require follow-up actions. <br />The UST Owner or Operator must maintain a copy of this monthly visual inspection checklist and all attachments for the <br />previous 12 months. The records must be maintained on-site or, if approved by the local agency, off-site at a readily <br />available location. <br />
