Printout from old TLS shows L4 trench fuel alarm on 1-8-08 @ <br />9:55pm caused when fuel line to engine burst and flooded trench. Documentation is not yet available. <br />New TLS alarm history shows system security warning on 1-15-08 @ 10:57am, T1 hi product alarm on 1-15-08 <br />11:52am, L2 annular sensor out alarm on 1-23-08 @ 11:46am, and L4 generator room trench fuel alarms on 1-108 at <br />11:36am and 11:58am. The alarms on 1-15-08 were caused during TLS installation and the L2 alarm on 1-23-08 was <br />caused during other tank repairs. Documentation is not yet available. Per AT&T Connie Mitchell, TLS and sensors have <br />not yet been certified. <br />Item #10 - Documentation is not available for SB989 secondary containment testing. <br />Item #11 - Spill bucket failed testing on 7-13-07 and is being replaced using ticket #LT -5209. Spill bucket has been <br />placed and is waiting inspection on 1-28-08. <br />Documentation is not available pertaining to L2 alarm on 8-3-07 or L3 alarm on 7-31-07. <br />#LT -5209 is open to: <br />- replace TLS 300C with TLS 350 Plus (waiting certification). <br />- replace spill bucket (waiting inspection). <br />- replace annular riser (work in progress). <br />Items Ke ulrin Follow -U Actions: <br />Obtain and post documentation for: <br />- SB989 secondary containment testing. <br />- L2 alarm activity on 8-3-07. <br />- L3 alarm activity on 7-31-07. <br />- TLS 350 installation and certification. <br />- spill bucket installation and testing. <br />- annular riser installation. <br />- L4 alarm activity on 1-8-08. <br />Instructions: <br />Monthly visual inspection of the UST system must be conducted by a Designated UST Operator, who <br />possesses a current California UST System Operator certification issued by the International Code Council. <br />A copy of this monthly visual inspection checklist must be provided to the UST Owner or Operator. <br />The Designated UST Operator must alert the UST Owner or Operator of any condition discovered during the monthly visual <br />inspection that may require follow-up actions. <br />The UST Owner or Operator must maintain a copy of this monthly visual inspection checklist and all attachments for the <br />previous 12 months. The records must be maintained on-site or, if approved by the local agency, off-site at a readily <br />available location. <br />
