The missing <br /> alarms include: <br /> -1-5-91 annular on 7/31/18. <br /> -L9-UDC 5/6 from 10/9/18 <br /> -L-11-UDC 10/7/18 <br /> Furthermore the designated operator failed to print out alarm L9 during the inspection report conducted on 7/6/18, <br /> 8/23/18 <br /> The designated operator marked"N/A"for"Are under dispenser containment areas free of water, debris,and <br /> hazardous substance?'and"Leak detection is properly located within under-dispense containment"during the <br /> 9/20/17, 10/20/17, 11/16/17, 2/21/18,and 4/27/18. <br /> -The designated operator failed to mark if"Are spill buckets(containment structures)free of water, debris, and <br /> hazardous substance?"for the 8/23/18. <br /> -The designated operator(DO)failed to include the CERS ID, the zip code of the site,failed to mark"is the <br /> containment sump free of damage,water, debris, and hazardous substance?"furthermore the DO failed to mark if <br /> "are all sensors in the under-dispenser containment located to detect a leak at the earliest opportunity?"in the <br /> 9/27/18 and 10/24/18 monthly inspection report. <br /> -The designated operator failed to check and note the correct test dates for the overfill prevention equipment <br /> inspection and secondary containment testing in the 9/27/18 and 10/24/18 inspection report. <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1(a) UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Fuel filters in UDC 1/2, 3/4,5/6,and 7/8 were observed outside of all under dispenser containments at the facility. <br /> The underground storage tank system shall be operated to prevent unauthorized releases. Immediately contact a <br /> properly licensed,trained, and certified contractor to replace or upgrade the component under permit and inspection <br /> with the EHD, if required. <br /> This is a Class II violation. <br /> 314 HSC 25290.1,25290.2,25291 Failed to have secondary containment or secondary containment not tight. <br /> The fuel filters in UDC 1/2,3/4, 5/6 and 7/8 are not contained within the secondary containment(under dispenser <br /> containment)for the dispenser piping.All primary portions of the UST system must be provided with adequate <br /> secondary containment.
