A. MATERIAL SAFETY DATA SHEETS (MSDS) <br />-- Enclosed. <br />B. SITE HISTORY <br />Site operates as a trucking facility of approximately 75 trucks. The on-site currently consists of (2) <br />20,000 gallon ea. underground storage tanks contained diesel. The tanks are manifolded together <br />and have piping to one dispenser. <br />Recently, fuel found in UST #l, annualar space. Consequently the owner removed all fuel from <br />the tank #1 and the tank is currently out of service. <br />UST #2 is in operating condition. <br />C. PROPOSED WORK. <br />The proposed work includes the removal of UST #1 and backfilling of the hole. It is also proposed <br />to upgrade of UST #2 for compliance with the 1998 underground tank guidelines which will <br />include double wall piping, drip pan for the dispenser and sumps for the submersible pumps. <br />D. FACILITY MAP <br />Enclosed. <br />E. WIND DIRECTION INDICATOR <br />Currently, we do not believe there will be a need of a wind indicator. Based on current site <br />conditions and known contaminant levels at the site, the ppe will be at level "D" will be <br />incorporated. Should the actual site conditions required the step up to a level "C", the need for a <br />wind direction indicator will be examined and a wind sock made available. <br />F . EXCAVATIONS <br />The excavation will be at maximum depth of 15' BGS with vertical sidewalls. The excavation will <br />closed off during all off -hours. <br />There will be a 6' high chain link fence surrounding the site to exclude unauthorized personnel. <br />If a person accidentally falls in the excavation the following procedures will be followed. <br />All work will cease, and all equipment will be stopped. The condition of the victim will be <br />determined. <br />If the victim is conscious and ambulatory, a ladder will be placed in the excavation for self <br />rescue. <br />If the victim is unconscious or not ambulatory, the Fire Dept. will contacted immediately <br />for rescue. <br />G.
