Financial responsibility documents have not been submitted to the EHD. A Certification of Financial Responsibility was not submitted. The insurance documents submitted are expired. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class II. 104 CCR 2711 UST Operating Permit Application for Facility and Tank information not submitted or current. The UST Tank Information form for the diesel tank is not current in CERS. The product piping is incorrectly listed as plastic primary with FRP secondary; the product piping observed on site was double walled fiberglass in the 1.5 hp turbine sump and steel in the 5 hp turbine sump. The vent lines are incorrectly listed as steel; the vent piping observed on ste was single walled fiberglass. The under dispenser containment (UDC) was listed as steel; the UDC observed on site was single walled plastic. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and/or not approved by the EHD. The monitoring plan has the following issues: - The annular sensor is listed as a Red Jacket FXIV; the annular sensor was observed to be a Veeder Root 420. - The plan indicates that there are not sump sensors; the sump sensors were observed to be Veeder Root 205. - The plan indicates that there is no positive shutdown during sensor out and power out; positive shutdown was verified for both conditions during the inspection. - The plan indicates that there are no UDCs; the UDC sensor was observed to be a Veeder Root 205. - The plan indicates that there are no leak detectors; the leak detectors were observed to be a FE Petro STP -MLD -D (1.5 hp) and a VMI 99LD3000 (5 hp) mechanical line leak detectors. - The plan incorrectly indicates that alarm logs are not being maintained on site. - No second person responsible is listed. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class ll. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The map submitted does not include the locations of sumps, sensors, dispensers, or the monitoring panel. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of a complete, accurate UST Monitoring Site Plan. This is a minor violation.
