The Statement of UST Designated <br />Operator submitted lists only Michael Young. Any changes shall be reported to the EHD within 30 days. Immediately <br />log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification <br />identifying all the designated operators for this facility. <br />This is a Class II violation. <br />204 <br />CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br />on site at all times. Immediately locate and retain a copy on site. <br />This is a minor violation. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the monitoring system certification and spill <br />bucket testing on the December 2014 designated operator monthly inspection report. The report incorrectly listed the <br />2013 test date. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required information on the <br />reports. <br />The designated operator failed to document all the alarms from the attached alarm history on the December 2014 and <br />February 2015 designated operator monthly inspection report and failed to check that they were responded to <br />appropriately. The missing alarms include: L2 (5 hp sump) fuel alarm on 2/19/15 and L4 (UDC) fuel alarm on 11/20/15. <br />The L4 alarm was noted, but the designated operator did not include any documentation regarding the response to the <br />alarm. Alarm history printouts were not attached to the monthly inspection reports. During the monthly inspection, <br />the designated operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report.
