Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by May 17, 2013. <br />105. UST facility and tank forms have not been submitted to the EHD. Any change of information on <br />these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and <br />submit a copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank. <br />106 & 202. Current approved copies of the monitoring and response plans were not on site. The <br />monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br />of the monitoring and response plans for approval by the EHD by May 17, 2013. An approved copy will <br />be returned for your records. <br />107. A written procedure for monitoring, submitted on the "Underground Storage Tank Monitoring Plan" <br />in Title 27, Division 3, Subdivision 1, Chapter 6, which establishes: The location(s), as identified on a plot <br />plan, where the monitoring be has not been submitted to the EHD. Complete and submit a copy of the <br />plot plan/site map that shows where the monitoring equipment is located. Submit the plot plan/site map <br />with the monitoring and response plans for approval by the EHD by May 17, 2013. An approved copy <br />will be returned for your records. <br />108. Secondary containment testing was last performed on February 23, 2010, and was completed on <br />March 25, 2013, one month past due. Secondary containment testing is required once every 36 months. <br />Take all necessary precautions to ensure testing is performed in a timely manner. <br />201.
