Hwy 88 Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />These records shall be kept on site for at least 3 years. Immediately locate and maintain the facility <br />alarm log on site and submit copies to the EHD by May 17, 2013. <br />204. The designated operator failed to check and note the correct test dates for the secondary <br />containment test on the July 2012 through April 2013 designated operator monthly inspection reports. <br />The test date was listed as March 4, 2010, but should have been listed as March 25, 2013. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the <br />UST system have been completed and document the dates they were done. Ensure that the designated <br />operator performing monthly inspections at this facility is including all of the required information on the <br />reports. <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />December 10, 2012, designated operator monthly inspection report and failed to check that they were <br />responded to appropriately. The missing alarm includes: L3 diesel turbine sump sensor alarm. The <br />designated operator listed a L3 diesel turbine sump sensor alarm for December 5, 2012, but did not list <br />the L3 diesel turbine sump sensor alarm for December 2, 2012. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was <br />documented and responded to appropriately, and attach a copy of the alarm history with documentation <br />taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />310. Liquid was observed in the 1/2 and 3/4 dispenser pans and in the 87 turbine sump. If liquid could <br />enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of <br />properly. Take the necessary precautions to ensure that the secondary containment of the UST system <br />is maintained free of liquid. During the inspection the technician removed the liquid from the pans/sump. <br />Continue to ensure that any liquid from the secondary containment sumps are managed properly per <br />California Title 22 Hazardous Waste Regulations. <br />313. The 87 direct -bury spill container failed when tested and had a maximum capacity of three and a <br />half gallons. All spill containers shall have a minimum capacity of five gallons and be capable of <br />containing a spill or overfill until it is detected or cleaned up. Immediately discontinue deposition of <br />petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, and <br />certified contractor under permit and inspection of the EHD. If the spill container can't be replaced <br />immediately, there is a possibility that the 87 UST system may be red tagged to prevent fuel inputs. <br />The inventory of 87 fuel at the time of the spill bucket failure was 3,766 gallons. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT T REINSPECTION AT ANY TIIqE AT END'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received <br />Title: <br />Aris Cacapit (209) 468-3453 ` jk <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
