Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. During the inspection Eddie Sandhu <br />submitted a current copy of the financial responsibility documents. <br />108. Secondary containment testing was scheduled to be performed on 2-23-10 and a test report was <br />submitted to the EHD on 4-5-10, one month late. A copy of the test report must be submitted within 30 <br />days of the test. Ensure that a copy of the test report is sent to the EHD within 30 days of the test. <br />109. Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on 4-25-11 and a test report was submitted to the EHD on 6-28-11, one month late. A copy of <br />the test report must be submitted within 30 days of the tests. Ensure that a copy of the test report is sent <br />to the EHD within 30 days of the test. <br />204. The designated operator failed to check and note the correct test dates for the monitoring <br />certification, leak detector, spill container and secondary containment testing on the July 2011 through <br />March 2012 designated operator monthly inspection reports. The designated operator failed to check <br />and note the correct test date for the secondary containment testing also on the June 2011 designated <br />operator monthly inspection report. The monitoring certification, leak detector, and spill container tests <br />were listed as 4-29-11. It should have listed as 4-25-11. The secondary containment test was listed as <br />3-1-10 and 3/2010. It should have been listed as 2-23-10. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed <br />and document the dates they were done. Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. <br />303. The 87 turbine sump sensor was raised approximately 12 inches off the lowest point of the sump <br />and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to <br />be able to detect a leak at the earliest possible opportunity. During the inspection, the technician <br />repositioned this sensor to the lowest point of the sump to detect a leak at the earliest opportunity. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />TFAS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD 23-02 Rev 09/01/11 Page 3 UST INSPECTION REPORT <br />
