LoCkeford, Lodi Inspection Aug, 2012 <br />Name: p Address: Date: g <br />SURYLARY OF <br />VIOLATIONS- <br />CLASS I CLASS I or INOR-No Com otice t 1 <br />This is a report of the Routine and underground storage _ta_nkm`6`ffiT5`r`ing system certification inspection <br />conducted at the referenced facility by San Joaquin County Environmental Health Department (EHD). <br />The following are Class 11 (Repeat) violations: 310, and the following are Minor Violations - Notice to <br />Com: 106, 110, 113, 204, 206 <br />106. Monitoring plan lists monitoring system as Veeder Root TLS -350, when the monitoring system was <br />observed as EMC Gibraco, and therefore the monitoring plan is not current. The monitoring and <br />response plans must be current and approved by the EHD. Complete and submit.a copy of the <br />monitoring plan for approval by the EHD by September 3, 2012. An approved copy will be returned for <br />your records. <br />110. Technicians were unable to test the 91 -octane line leak detector during this inspection; the leak <br />detector was last tested on August 5, 2011. Leak detectors shall be tested annually, and notification <br />provided to the EHD at least 48 hours in advance of testing. Immediately schedule the test before <br />August 31, 2012, and provide 48 hours notification to the EHD for a re -inspection. Please be aware this <br />re -inspection will be billed at current EHD hourly rate. <br />113. Christopher Kukendall signed off as the designated operator on the July 11, 2012 designated <br />operator inspection report, but he is not listed as one of the designated operators for the facility. Any <br />changes of designated operator shall be reported to the EHD with 30 days. Submit notification to the <br />EHD identifying all the designated operators for this facility by September 3, 2012. <br />204 The designated operator failed to verify that all employees (new employee Ravi Singh, see Item <br />206 below) have been trained during the July 2012 monthly inspection. During the monthly inspection, <br />the designated operator shall verify that all facility employees have been trained.
