The designated operator initial employee training for Ravi Singh who was hired about one month ago based on an interview with the employee, was not performed. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD by September 3, 2012. 310 (Repeat). About quarter gallon of liquid was observed in the 91 -octane STP sump, 2.5 cups in the 87 -octane STP sump, 1 cup in the 89 -octane STP sump, 1 cup in the 87 -octane fill sump, and 1.5 to 2 gallons in the 91 -octane fill sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Technicians was unable to remove the liquid during this inspection, but stated they will return and remove the liquid at a later time. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the above sumps are maintained free of liquid. Schedule an inspection with EHD, or submit documents that will verify the disposal of the liquid to EHD by September 3, 2012. NOTES The 91 -octane spill container failed when first tested, but passed after the cap and swivel was replaced by technicians on site. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by September 3, 2012. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
