The designated operator initial employee training for Ravi Singh who was hired about one month <br />ago based on an interview with the employee, was not performed. The designated operator shall train <br />facility employees for which he or she is responsible in the proper operation and maintenance of the <br />UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by September 3, 2012. <br />310 (Repeat). About quarter gallon of liquid was observed in the 91 -octane STP sump, 2.5 cups in the <br />87 -octane STP sump, 1 cup in the 89 -octane STP sump, 1 cup in the 87 -octane fill sump, and 1.5 to 2 <br />gallons in the 91 -octane fill sump. If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. Technicians was unable to remove the liquid <br />during this inspection, but stated they will return and remove the liquid at a later time. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br />and manage it accordingly. Ensure that the above sumps are maintained free of liquid. Schedule an <br />inspection with EHD, or submit documents that will verify the disposal of the liquid to EHD by September <br />3, 2012. <br />NOTES <br />The 91 -octane spill container failed when first tested, but passed after the cap and swivel was replaced <br />by technicians on site. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by September 3, 2012. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Rec ' ed By: <br />Title: <br />Jeffrey Wong (209) 468-0335 <br />EHD 23-02-003 Rev 04/19/12 CV4 INUATION FORM <br />
