The pelma has leen approved with the <br />folknuing conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Thuy Tran at (299) 468-6257 to arrange the following inspection appointments <br />(a 46 -hour notice Is required for inspection appointments). Pksase be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a Corrosion protection: Verify corrosion protection for dined bury so bucket priorto <br />backfilling. <br />b. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />I" <br />(or equivalent). Proper drainage must also be verified. <br />a. Secondary containment testing on 91 VST. <br />Please be aware that the facility cannot receive fuel in the 91 tank prior to the EMD <br />[nspection and approval <br />4. Since Concrete wig be broken rear the 91 UST, a secondary containment lest must be <br />performed on the 91 UST once concrete is poured In accordance with the <br />manufacturer's guidelines. <br />5. Qualified service technicians must possess orwork under the direct and personal <br />supervision of an individual physical presont at the work site who possesses all of the <br />following: <br />
