The permit has been approved with the <br />following conditions: <br />- .-.. - • - ... - -. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br />backfilling. <br />b. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />c. Secondary containment testing on 91 UST. <br />Please be aware that the facility cannot receive fuel in the 91 tank prior to the EHD <br />inspection and approval. <br />4. Since concrete will be broken near the 91 UST, a secondary containment test must be <br />performed on the 91 UST once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
