UST Tank Information forms for the diesel tank are not current in CERS. For Tank ID#:TA0507917 (diesel), please list the primary containment. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The response plan is not current and has not been uploaded into the California Environmental Reporting System (CERS) for approval by the EHD. The response plan must be uploaded to the CERS. Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a Class II violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a Class II violation. 115 CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. A new designated operator was hired on 5-19-14(Ron Rocha), 7-16-14(Ruben Galindo), and 2-24-15(Jose Ochoa) and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. Note: Jacob Weaver was hired on 3-24-15, and the EHD must be notified by 4-24-15- This is a Class II violation. 126 HSC 25404(e)(4) Failed to report program data electronically. Beginning January 1, 2014, all businesses are required to submit all new (or any changes to existing) UST information, including: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator Written Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance online to the CERS at http://cers.calepa.ca.gov.
