The monitoring system certification, leak detector, and spill container testing were listed <br /> as 4-29-14. These should have been listed as 4-21-14. The secondary containment test was listed as 7-15-12. This <br /> should have been listed as 7-11-12. <br /> The designated operator failed to check and note the correct test dates for the secondary containment, monitoring <br /> system certification, and spill container testing on the August 2014 through January 2015 designated operator monthly <br /> inspection reports. The monitoring system certification and spill container testing were listed as 4-29-14. These should <br /> have been listed as 4-21-14. The secondary containment test was listed as 7-15-12. This should have been listed as <br /> 7-11-12. <br /> The designated operator failed to check and note the correct test dates for the secondary containment testing on the <br /> May through July 2014 designated operator monthly inspection reports. The secondary containment test was listed as <br /> 8-3-11. This should have been listed as 7-11-12. <br /> During the monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The diesel line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a <br /> 3-gallon per hour leak at 10 psi. The leak detector was replaced and retested during the inspection. A"UST Retrofit <br /> Verification with Inspector Already Onsite" form has been completed and provided to the operator and contractor. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 5-20-15. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
