The designated operator failed to check and note the correct test dates for the secondary containment, monitoring system certification, leak detector, and spill container testing on the February and March 2015 designated operator monthly inspection reports. The monitoring system certification, leak detector, and spill container testing were listed as 4-29-14. These should have been listed as 4-21-14. The secondary containment test was listed as 7-15-12. This should have been listed as 7-11-12. The designated operator failed to check and note the correct test dates for the secondary containment, monitoring system certification, and spill container testing on the August 2014 through January 2015 designated operator monthly inspection reports. The monitoring system certification and spill container testing were listed as 4-29-14. These should have been listed as 4-21-14. The secondary containment test was listed as 7-15-12. This should have been listed as 7-11-12. The designated operator failed to check and note the correct test dates for the secondary containment testing on the May through July 2014 designated operator monthly inspection reports. The secondary containment test was listed as 8-3-11. This should have been listed as 7-11-12. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The diesel line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. The leak detector was replaced and retested during the inspection. A"UST Retrofit Verification with Inspector Already Onsite" form has been completed and provided to the operator and contractor. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 5-20-15. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance.
