Mr. Cacapit also found that current financial responsibility <br />documents had not been submitted to the EHD. <br />On September 9, 2008, a permit application, SR0055348, was submitted by Elite IV to <br />replace the 87 -octane leak detector and the drain valve on the 91 -octane spill container. <br />This permit was approved by Mr. Cacapit on September 15, 2008 (Attachment 20). <br />On October 1, 2008, Mr. Cacapit witnessed the successful testing of the 87 -octane leak <br />detector and the 91 -octane spill container (Attachment 21). <br />On October 1, 2008, a return to compliance certification for the September 8, 2008, UST <br />inspection was submitted that included a corrective actions statement and financial <br />responsibility documents (Attachment 22). <br />On January 23, 2009, secondary containment testing was performed and all <br />components tested passed (Attachment 23). <br />On May 1, 2009, a permit application, SR0057217, was submitted by Franzen -Hill <br />Corporation (Franzen -Hill) to install a Healy bladder tank, a Healy VP -1000 retrofit kit to <br />the existing dispensers, and add a vent line to the building (Attachment 24). Ms. Henry <br />spoke to Ms. Hope Vargas of Franzen -Hill and explained to her the deficiencies that <br />needed to be addressed before the permit would be approved. <br />On May 18, 2009, a permit application, SR0057330, was submitted by Stockton Service <br />Station Equipment Company, Inc. (SSSE) to replace the main board in the monitoring <br />panel. This permit was approved by Mr. Jeffrey Wong, Senior REHS, EHD, on May 27, <br />2009 (Attachment 25). <br />On June 10, 2009, Mr. Wong called Franzen -Hill and spoke with a woman named <br />Summers (full name unknown) and asked if they had submitted the information <br />requested for permit application SR0057217. Summers stated that she would speak <br />with Ms. Vargas. <br />On June 12, 2009, Mr. Wong sent a letter to Franzen -Hill listing the deficiencies of <br />permit application SR0057217 that needed to be addressed before it could be approved <br />(Attachment 26). <br />On June 16, 2009, Mr. Wong witnessed the testing of all the sensors after the <br />replacement of the main board in the monitoring panel (Attachment 27). Mr. Eric <br />Molgaard, a service technician with SSSE, estimated that the panel failed around March <br />19, 2009, based on the information he gathered from the monitoring panel's alarm <br />history and monthly designated operator reports. <br />On August 20, 2009, the designated operator for this facility, Mr. Dan McIlrath, called <br />and asked about the status of permit application SR0057217. Mr. Wong explained that <br />there were deficiencies in the application that needed to be addressed before it could be <br />approved. <br />
