On September 24, 2009, a permit application, SR0058393, was submitted by Franzen - Hill to replace the existing Incon TS -1000 monitoring panel with an Incon TS -550 monitoring panel. This permit was approved on October 1, 2009 (Attachment 28). On September 29, 2009, Mr. Wong was on site to witness a monitoring system certification scheduled by Afforda-Test. Upon arrival, Mr. Wong found that the service technician had not arrived. Mr. Wong called Afforda-Test and was told that the testing was cancelled about a week ago because the monitoring panel was down. Mr. Wong issued a Notice to Abate to the owner, Mr. Heng Chea, requiring monitoring to be restored (Attachment 29). Mr. Molgaard arrived on site at the request of Mr. Chea. Mr. Molgaard stated that he was on site about two weeks prior, after a power outage caused the monitoring panel to go into alarm, and that the panel needed to be reprogrammed. The sensor set up was still intact. After reviewing the system setup with Mr. Wong, Mr. Molgaard stated that the sensor setup was currently not properly programmed and that there was no monitoring of the UST system. Mr. Chea asked SSSE to make the necessary repairs. Mr. Molgaard stated he could not do so that day, but he would return the following morning. A permit application, SR0058424, was submitted by SSSS to reprogram the monitoring panel and approved by Mr. Wong on September 30, 2009 (Attachment 30). On September 29, 2009, Ms. Diane Lopez of Franzen -Hill called and asked Mr. Wong if Mr. Chea hired another contractor to repair the monitoring system. Mr. Wong did not answer her question directly but he stated that the EHD has no comment if there was a contract dispute. Mr. Wong asked her about the information required to be submitted before permit SR0057217 would be approved. Ms. Lopez stated that she had not sent in any of the required information because she was waiting on permits from the local Fire and Building departments. On September 30, 2009, Mr. Wong witnessed the successful testing of all the sensors, sensor out, and fail safe under permit SR0058424. Monitoring had been restored and Mr. Wong left an inspection report with Mr. Chea (Attachment 31). On October 7, 2009, Mr. Wong witnessed the successful testing of the sensors, sensor out, and fail safe after replacement of the monitoring panel under permit SR0058393 (Attachment 32). Franzen -Hill submitted a test report for this work on October 13, 2009, incorrectly identifying it as the annual monitoring system certification (Attachment 33). On February 9, 2010, Mr. Wong spoke to Mr. Chea, who stated that an EVIR upgrade on the UST system was done in September 2009. Mr. Wong explained that the EHD never issued an approval for permit application SR0057217 to perform this work, and the information requested in the June 12, 2009, deficiency letter sent to him and Franzen - Hill had not been received. Mr. Chea stated that it was not his responsibility and that Mr. Wong should talk to Franzen -Hill. Mr. Wong called Franzen -Hill and spoke with Ms. Lopez who admitted they completed the work for the EVIR upgrade on the UST system on December 18, 2009. Mr. Wong explained that the EHD never issued an approval for the permit, and Ms. Lopez stated that she would call back after gathering all her documents. Ms. Lopez submitted a copy of an EHD stamp on the front page of a set of plans submitted for a building permit with the comment, "No objections for EVIR (Enhanced 0
