On September 24, 2009, a permit application, SR0058393, was submitted by Franzen - <br />Hill to replace the existing Incon TS -1000 monitoring panel with an Incon TS -550 <br />monitoring panel. This permit was approved on October 1, 2009 (Attachment 28). <br />On September 29, 2009, Mr. Wong was on site to witness a monitoring system <br />certification scheduled by Afforda-Test. Upon arrival, Mr. Wong found that the service <br />technician had not arrived. Mr. Wong called Afforda-Test and was told that the testing <br />was cancelled about a week ago because the monitoring panel was down. Mr. Wong <br />issued a Notice to Abate to the owner, Mr. Heng Chea, requiring monitoring to be <br />restored (Attachment 29). Mr. Molgaard arrived on site at the request of Mr. Chea. Mr. <br />Molgaard stated that he was on site about two weeks prior, after a power outage caused <br />the monitoring panel to go into alarm, and that the panel needed to be reprogrammed. <br />The sensor set up was still intact. After reviewing the system setup with Mr. Wong, Mr. <br />Molgaard stated that the sensor setup was currently not properly programmed and that <br />there was no monitoring of the UST system. Mr. Chea asked SSSE to make the <br />necessary repairs. Mr. Molgaard stated he could not do so that day, but he would return <br />the following morning. A permit application, SR0058424, was submitted by SSSS to <br />reprogram the monitoring panel and approved by Mr. Wong on September 30, 2009 <br />(Attachment 30). <br />On September 29, 2009, Ms. Diane Lopez of Franzen -Hill called and asked Mr. Wong if <br />Mr. Chea hired another contractor to repair the monitoring system. Mr. Wong did not <br />answer her question directly but he stated that the EHD has no comment if there was a <br />contract dispute. Mr. Wong asked her about the information required to be submitted <br />before permit SR0057217 would be approved. Ms. Lopez stated that she had not sent <br />in any of the required information because she was waiting on permits from the local Fire <br />and Building departments. <br />On September 30, 2009, Mr. Wong witnessed the successful testing of all the sensors, <br />sensor out, and fail safe under permit SR0058424. Monitoring had been restored and <br />Mr. Wong left an inspection report with Mr. Chea (Attachment 31). <br />On October 7, 2009, Mr. Wong witnessed the successful testing of the sensors, sensor <br />out, and fail safe after replacement of the monitoring panel under permit SR0058393 <br />(Attachment 32). Franzen -Hill submitted a test report for this work on October 13, 2009, <br />incorrectly identifying it as the annual monitoring system certification (Attachment 33). <br />On February 9, 2010, Mr. Wong spoke to Mr. Chea, who stated that an EVIR upgrade on <br />the UST system was done in September 2009. Mr. Wong explained that the EHD never <br />issued an approval for permit application SR0057217 to perform this work, and the <br />information requested in the June 12, 2009, deficiency letter sent to him and Franzen - <br />Hill had not been received. Mr. Chea stated that it was not his responsibility and that Mr. <br />Wong should talk to Franzen -Hill. <br />Mr. Wong called Franzen -Hill and spoke with Ms. Lopez who admitted they completed <br />the work for the EVIR upgrade on the UST system on December 18, 2009. Mr. Wong <br />explained that the EHD never issued an approval for the permit, and Ms. Lopez stated <br />that she would call back after gathering all her documents. <br />Ms. Lopez submitted a copy of an EHD stamp on the front page of a set of plans <br />submitted for a building permit with the comment, "No objections for EVIR (Enhanced <br />0 <br />
