Ms. Lopez <br />claimed that this was the approval she received for the EVR permit from the EHD. <br />On February 11, 2010, Mr. Wong drove by the facility and took photos of what appeared <br />to be newly installed equipment (Attachment 35). <br />On February 26, 2010, Ms. Lopez called and left a voicemail for Mr. Wong asking to <br />schedule an inspection for permit SR0057217. Mr. Wong called back and stated that <br />there was no inspection because the permit was never approved. Ms. Lopez began to <br />get angry with Mr. Wong and wanted to speak to his supervisor. Mr. Wong gave Ms. <br />Lopez the contact information for Ms. Kasey Foley, REHS, Program Coordinator, EHD. <br />On March 1, 2010, Ms. Lopez called Ms. Foley to discuss not getting the permit <br />approval. Ms. Lopez explained that they did not install a Healy system as originally <br />stated in the permit application, but installed a Hirt system instead. Ms. Foley asked Ms. <br />Lopez to resubmit the permit application with plans to Mr. Wong. <br />On March 4, 2010, a revised permit application SR0057217 was submitted by Franzen - <br />Hill (Attachment 36). The application indicated the equipment that was installed, but the <br />accompanying site map did not accurately depict the site. The site map showed three <br />previously removed USTs in their former location, dispensers on the wrong side of the <br />canopy posts with conduit trenching going straight to one of them from the building, and <br />did not show the existing USTs or the UST piping. <br />On March 11, 2010, Mr. Wong called Ms. Lopez and explained the deficiencies in the <br />site map that needed to be addressed. Ms. Lopez stated that she would have Mr. <br />Richard Warren of Franzen -Hill contactj Mr. Wong to address the deficiencies in person <br />at the EHD office by Monday, March 15, 2010, since he was scheduled to be in the area. <br />On March 16, 2010, Ms. Lopez called Mr. Wong and left a message stating that Mr. <br />Warren would be at the EHD office on Monday, March 22, 2010, to make the necessary <br />revisions to the site map and address any other discrepancies. Mr. Wong called Ms. <br />Lopez back and left a message stating that date was acceptable, and reiterated what <br />clarifications were needed. <br />On March 18, 2010, Mr. Wong spoke with Mr. Warren and confirmed the appointment on <br />March 22, 2010. Mr. Wong reemphasized that the site map was inaccurate and it <br />needed to clearly show the location of the equipment and that he may have to redraw <br />the site map if it was not clear. Mr. Warren said he understood. <br />On March 22, 2010, Mr. Wong met Mr. Warren at the EHD office. Mr. Wong explained <br />the items on the map that needed to be revised and gave them back to Mr. Warren. Mr. <br />Warren revised the permit application to indicate no cold start of the monitoring panel. <br />On March 23, 2010, Mr. Wong called and reminded Mr. Warren to provide three copies <br />of the site maps and to ensure it showed the locations of the LIST piping to determine if <br />secondary containment testing would be required if trenching was done over them. Mr. <br />Warren stated he would have it done later that week. <br />R <br />
