The following are Repeat Class II Violations: 201, <br /> 204,310,313 and Minor Violations - Notice to Comply : 203, 306 <br /> 203. About 2 cups of fuel release in the sump of the Bio-diesel Shed was not recorded. Any release <br /> from the primary containment that does not escape the secondary containment and was cleaned up <br /> within eight hours of being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records. Submit a copy of the recordable release statement to the EHD by June 14, 2014. <br /> 201 (Repeat). Maintenance/Service record responding to L6 alarm on 2/27/2014 was not found on site. <br /> These records shall be kept on site for at least 3 years. Immediately locate and maintain the on site <br /> and submit copies to the EHD by June 14, 2014. <br /> 204 (Repeat). The designated operator failed to document all the alarms from the attached alarm history <br /> on the designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: L6 alarm on 2/27/2014. During the monthly inspection, the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was <br /> documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report.
