The previous test was conducted on 7-27-2011 and was due by <br /> 8-27-2014. The secondary containment testing was conducted on-11 (passing results for UDC-13, UDC <br /> 14-15, and UDC-16 only were received), and on 8-19-14 for al] remaining components with exception of UDC-13, <br /> UDC 14-15, and UDC-16 (tested on 11-06-13). Secondary containment testing is required once every 36 months. <br /> The secondary containment testing for UDC-13, UDC 1471`5, and UDC-16 is is due by 11-16-2016 and for the rest <br /> of the components by 7-27-17. Take all necessary precp6tions to ensure testing is performed in a timely manner. <br /> This is a Class II violation. <br /> 123 CCR 2661 Permit to repair UST system not obtained. <br /> The concrete was broken in the vicinity of the"auto site" UDCs and/ed <br /> an a permit was not obtained from <br /> the EHD. Emergency repairs relating to UST system monitoring anection may be performed by a <br /> properly licensed, trained, and certified contractor pursuant to the Eand a permit obtained within one <br /> business day from the EHD. All other repairs must have a permit sand approved by the EHD prior to <br /> beginning work. Immediately obtain a permit from the EHD for theAn incomplete permit application was <br /> received on 5-6-16; immediately submit the complete permit applicEHD. A"Failure to Obtain a Permit" <br /> penalty applies. <br /> This is a Class II violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. The monitoring plan uploaded into <br /> CERS for the onsite oil water separator is not current and/or not approved by the EHD. The monitoring plan for the <br /> onsite oil water separator indicates that the leak alarm triggers the automatic pump shut down and failure or <br /> disconnection of the sensor also triggers an automatic pump shut down. A copy of these plans shall be accessible <br /> on site at all times. Immediately make the necessary corrections and retain a copy on site. <br /> This is a Class II violation. <br /> 205 CCR 2712(i) Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current and/or not approved by the EHD. The monitoring plan for the onsite oil water <br /> separator indicates that the leak alarm triggers the automatic pump shut down and failure or disconnection of the <br /> sensor also triggers an automatic pump shut down. The monitoring plan must be uploaded to the California <br /> Environmental Reporting System (CERS).
