The Diesel 3 tank annular sensor failed to activate an audible and visual alarm when teste nd, therefore, the tank is not monitored. The interstitial space shall be monitored with a monitoring system conn cted to an audible and visual alarm system. The sensor was replaced and retested during the inspection. A"YST Retrofit Verification with Inspector Already Onsite" form has been completed and provided to the operator and ontractor. This was corrected on site. This is a repeat violation, Class II.
