Oily liquid was also ted inside both biodiesel transition sump on <br /> above inspection dates. Any release from the primary containme that does not escape the secondary containment <br /> and was cleaned up within eight hours of being detected shall recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean t4release <br /> 4. Method and location of disposal of thyeaerd <br /> bstances <br /> 5. Description of actions taken to repairuture releases <br /> 6. Description of method used to reactivl monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release with the infired, and maintain it on site with the facility's monitoring <br /> records. Submit a copy of the recordable rent to the EHD. <br /> This is a repeat violation, Class IL <br /> 09 CCR 2715(c) Designated operator did not inspect all requir/na <br /> and/or th inspection reports not completed. <br /> The designated operator failed to check and note the corretes for a secondary containment testing on all <br /> reviewed 2015-2016 designated operator monthly inspectioThe econdary containment test was listed on <br /> the reviewed DO reports as 7-27-2014; however, the seconta ment testing for UDC-13, UDC 14-15, and <br /> UDC-16 was conducted on 11-06-2013 and for the rest of thents on 8-19-14. During the monthly <br /> inspection, the designated operator shall check that all requing and maintenance for the UST system have <br /> been completed and document the dates they were done. hat designated operators performing monthly <br /> inspections at this facility are including all of the required inon the reports. <br /> This is a repeat violation, Class Il. <br /> 305 HSC 25290.1(e)VPH monitoring of the interstitial spaces of t UST system is not maintained. <br /> The"south" biodiesel transition sump had a hose and a valv with a gauge attached to the end of the brine test port, <br /> when the valve was opened (and pressure released)the bri a level dropped significantly in the brine sensor holder <br /> and had to be replenished. The sensors monitoring the liq d level in the interstitial space shall be maintained so <br /> that a breach in the primary or secondary containment is tected before the hazardous substance is released into <br /> the environment. The hose and the gauge were removed y the technician onsite at the time of inspection, ensure <br /> that all interstitial monitoring is being maintained as requi d. <br /> This is a repeat violation, Class II. <br /> 307 CCR 2632(c) Monitoring system of the interstitial space of a double walled system not maint ' ed. <br /> The Diesel 3 tank annular sensor failed to activate an audible and visual alarm when teste nd, therefore, the tank <br /> is not monitored. The interstitial space shall be monitored with a monitoring system conn cted to an audible and <br /> visual alarm system. The sensor was replaced and retested during the inspection. A"YST Retrofit Verification with <br /> Inspector Already Onsite" form has been completed and provided to the operator and ontractor.
