Oily liquid was also noted inside both biodiesel transition sump on <br />above inspection dates. Any release from the primary containment that does not escape the secondary containment <br />and was cleaned up within eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br />containment <br />Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br />records. Submit a copy of the recordable release statement to the EHD. <br />This is a repeat violation, Class II. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the secondary containment testing on all <br />reviewed 2015-2016 designated operator monthly inspection report. The secondary containment test was listed on the <br />reviewed DO reports as 7-27-2014; however, the secondary containment testing for UDC -13, UDC 14-15, and UDC -16 <br />was conducted on 11-06-2013 and for the rest of the components on 8-19-14. During the monthly inspection, the <br />designated operator shall check that all required testing and maintenance for the UST system have been completed <br />and document the dates they were done. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />305 <br />HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained. <br />The "south" biodiesel transition sump had a hose and a valve with a gauge attached to the end of the brine test port, <br />when the valve was opened (and pressure released) the brine level dropped significantly in the brine sensor holder and <br />had to be replenished. The sensors monitoring the liquid level in the interstitial space shall be maintained so that a <br />breach in the primary or secondary containment is detected before the hazardous substance is released into the <br />environment. The hose and the gauge were removed by the technician onsite at the time of inspection, ensure that all <br />interstitial monitoring is being maintained as required. <br />This is a repeat violation, Class II. <br />Page 6 of 7 <br />
