On May 4, 2016 and on May 5, 2016, oily staining was noted on the floor (around the suction pump) and pooling of oily liquid was noted inside the Biodiesel sump. Oily liquid was also noted inside both biodiesel transition sump on above inspection dates. Any release from the primary containment that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following information: 1. Operator's name and phone number 2. List of types, quantities, and concentrations of hazardous substances released 3. Description of actions taken to control and clean the release 4. Method and location of disposal of the released substances 5. Description of actions taken to repair and prevent future releases 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment Immediately record this release with the information required, and maintain it on site with the facility's monitoring records. Submit a copy of the recordable release statement to the EHD. This is a repeat violation, Class II. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment testing on all reviewed 2015-2016 designated operator monthly inspection report. The secondary containment test was listed on the reviewed DO reports as 7-27-2014; however, the secondary containment testing for UDC -13, UDC 14-15, and UDC -16 was conducted on 11-06-2013 and for the rest of the components on 8-19-14. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 305 HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained. The "south" biodiesel transition sump had a hose and a valve with a gauge attached to the end of the brine test port, when the valve was opened (and pressure released) the brine level dropped significantly in the brine sensor holder and had to be replenished. The sensors monitoring the liquid level in the interstitial space shall be maintained so that a breach in the primary or secondary containment is detected before the hazardous substance is released into the environment. The hose and the gauge were removed by the technician onsite at the time of inspection, ensure that all interstitial monitoring is being maintained as required. This is a repeat violation, Class II.
