The monitoring plan for the onsite oil water <br />separator indicates that the leak alarm triggers the automatic pump shut down and failure or disconnection of the <br />sensor also triggers an automatic pump shut down. The monitoring plan must be uploaded to the California <br />Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for <br />review by the EHD. <br />This is a repeat violation, Class Il. <br />109 <br />CCR 2637 Failed to test secondary containment. <br />Secondary containment testing was performed late. The previous test was conducted on 7-27-2011 and was due by <br />8-27-2014. The secondary containment testing was conducted on 11-06-13 (passing results for UDC -13, UDC 14-15, <br />and UDC -16 only were received), and on 8-19-14 for all remaining components with exception of UDC -13, UDC 14-15, <br />and UDC -16 (tested on 11-06-13). Secondary containment testing is required once every 36 months. The secondary <br />containment testing for UDC -13, UDC 14-15, and UDC -16 is is due by 11-16-2016 and for the rest of the components <br />by 7-27-17. Take all necessary precautions to ensure testing is performed in a timely manner. <br />This is a Class II violation. <br />123 <br />CCR 2661 Permit to repair UST system not obtained. <br />The concrete was broken in the vicinity of the "auto site" UDCs and fuel piping and a permit was not obtained from the <br />EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly <br />licensed, trained, and certified contractor pursuant to the EHUS policy and a permit obtained within one business day <br />from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. <br />Immediately obtain a permit from the EHD for these repairs. An incomplete permit application was received on 5-6-16; <br />immediately submit the complete permit application to the EHD. A "Failure to Obtain a Permit" penalty applies. <br />This is a Class II violation. <br />204 <br />CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. The monitoring plan uploaded into CERS <br />for the onsite oil water separator is not current and/or not approved by the EHD. The monitoring plan for the onsite oil <br />water separator indicates that the leak alarm triggers the automatic pump shut down and failure or disconnection of <br />the sensor also triggers an automatic pump shut down.
