Provide 48 hours notification to the EHD when this test is scheduled. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> 6. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, <br /> and <br /> b. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International <br /> Code Council (ICC). <br /> 7. All components must be certified by an independent testing organization and labeled as <br /> required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br /> Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br /> 8. A "UST Certification of Installation/Modification" form shall be submitted by the owner or <br /> the owner's agent [CCR, section 2635(d)] to the EHD or log into the California <br /> Environmental Reporting System (CERS) and upload the form before this secondary <br /> containment upgrade will not be considered complete by the EHD. <br /> 9. During installation ensure that the workspace is operating below the lower explosive limit <br /> (LEL), the workspace has to be tested regularly during installation or monitored <br /> continuously, per S Bravo installation instructions. Prior to beginning of work, provide <br /> LEL calibration records to the EHD. <br /> 10. A confined-space certification is required when installing the proposed fittings inside tank <br /> sumps, per manufacturer's installation instructions. <br /> 11. Permit expires 180 days from the date of approval.
