All non-Fillner employees will be required to sign in with the site supervisor prior to entering <br />the site. The site supervisor, Nahum Valdez, will answer any questions or concerns raised by <br />anyone visiting or working at the site. <br />In accordance with Fillner Construction Inc. policy and CAL -OSHA regulations, weekly tailgate <br />safety meetings shall be held on site with all personnel. Topics of discussion will include the regularly <br />scheduled topic of the week and all site specific concerns expected to be encountered in the course <br />of performing the various tasks scheduled. In addition, safety meetings will be held prior to beginning <br />any task of special concern for a particular hazard or a change in site conditions. <br />The following tasks and additional personal protective equipment are identified as a special <br />concern on this site: <br />• HEAVY EQUIPMENT OPERATIONS <br />• ppe= hearing protection, safety glasses, gloves <br />® LIFTING AND MATERIAL HANDLING <br />• ppe= hearing protection, safety glasses, gloves, back brace <br />■ INSTALLATION OF AST/UST's <br />• ppe= hearing protection, safety glasses, gloves, LEL / 02 meter, fire <br />extinguishers <br />® TRENCHING AND EXCAVATING <br />• ppe= hearing protection, safety glasses, gloves <br />® CONFINED SPACES <br />® ppe= hearing protection, safety glasses, gloves, LEL / 02 meter, tripod & <br />harness <br />EMERGENCY RESPONSE PLAN <br />The Site Supervisor is responsible for implementing the site safety plan and any required <br />emergency response and is designated as the Lead Person in the event of an emergency. This <br />responsibility falls to the next senior employee at the job site in the absence of the Site Supervisor. <br />In the event of an emergency situation, the Lead Person shall evaluate if they have the <br />means to correct the condition causing the emergency. In the event that the appropriate personnel <br />and/or equipment required to correct the emergency condition are not at the job site, the Lead <br />Person or first responder shall telephone 911 for emergency help. In addition, the resident <br />Construction Engineer, the appropriate agencies, and contacts from the "Emergency Notification <br />Listings" will be immediately informed of the incident and the planned response. <br />All personnel shall be evacuated from the affected area, and moved a safe distance away <br />from the hazard. Site personnel and equipment shall be used to prevent other people from entering <br />the affected area until help arrives. NO ONE SHALL ENDANGER THEIR OWN LIFE BY <br />ENTERING A KNOWN HAZARDOUS AREA WITHOUT PROPER TRAINING, EQUIPMENT AND <br />SUPPORT REGARDLESS OF THE REASON. The Lead Person shall identify the area affected by <br />the emergency condition, and determine what constitutes a safe distance until professional <br />emergency personnel arrive at the site.
