Secondary containment testing for the UDC 13, 14/15, 16/17, 21 main, 21 satellite, and <br /> diesel line between UDC 20 and 21 are required six months after installation. Provide <br /> 48 hours notification to the EHD when this test is scheduled. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested, repaired, or serviced. <br /> 7. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, <br /> and <br /> b. A valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International <br /> Code Council (ICC). <br /> 8. Update and submit a copy of the "UST Monitoring Plan"for EHD approval. An approved <br /> copy will be returned for on site records. <br /> 9. A"UST Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owner's agent(CCR Section 2635(d)) before this upgrade is considered complete by <br /> EHD. <br /> 10. All components must be certified by an independent testing organization and labeled as <br /> required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br /> Regulations (CCR)Title 23, Chapter 16, Table B for a complete list. <br /> 11. Permit expires 180 days from the date of approval.
